Thank you for volunteering with LDYFF Football & Cheer! All coaches are required to submit their clearances before participating in any team activities. Please use this form to upload your current PA Background Check, Child Abuse Clearance, and FBI Clearance (if applicable). 4 Required Steps for Coaches and anyone with continual or direct youth contact:
(clearances info is good for 5 years and every year a signed disclosure form is needed)
Step1:
PA State Police Criminal Record Check:

Printout will be available online
Step2:
PA Child Abuse Clearance:
https://www.compass.state.pa.us/
Create New Account (if a first-time user)
Individual Log on (if reprinting or renewing clearance)
Clearance will be delivered by mail and later available online.
Step3:
FBI Clearance only if required if you have lived in PA Less Than 10 years.

Step4:
Once you have all items please go to the following link to submit them to our organization.
